What to expect at your first visit
Congratulations for making your health a priority! We are glad that you are planning on visiting our office. From the first visit, expect a warm welcome, friendly service, and personalized, professional chiropractic care. Our goal is to educate you about your health concerns and treat the cause of your condition to help you reach and maintain optimal health and well-being. When you come into our office, you will be greeted by our receptionist, who will give you a brief tour of our office. She will then ask you to fill out some standard patient paperwork and she will ask to make a copy of you insurance card, if you have one. Once you complete your paperwork, Dr. Baldia will take you into his room, where he will do a thorough consultation. He will then do a chiropractic evaluation to help determine the source of your problem, and he will let you know if and how he can help you.
New Patient Paperwork
When you come for your first visit, please bring your insurance card so we may check your benefits for you. If you have any x-rays, MRIs, or records that you would like to show the doctor, please bring that as well. You may also want to print the forms below and fill them prior to your first appointment to save you some time.

We are in network with most major insurances including: Excellus BCBS, United Healthcare, Aetna, Smart Health, Empire Plan, Medicare, MVP, Multiplan, Cigna, Pomco, and Optum Health. We also accept Workers Compensation and Personal Injury.
